Prime Care Services is one of the Twin Cities’ trusted organizations offering person-centered, individualized supports to members of our communities coping with mental illness and disability.

We want to empower them with personal and professional skills to lead an independent life. We have years of experience in this field and offer expert guidance and support.

Trusted Support Services

We have a team of trained Direct Support Professionals who provide high-quality services and support to individuals living in their own homes throughout the Twin Cities. Currently, our services are available to individuals located in Dakota, Hennepin, Ramsey, and Washington Counties who receive CADI, DD, AC, and Elderly Waiver Services.

Our other services are:

  • Personal Supports (PS)
  • Respite (RP)
  • Independent Home Support (IHS)
  • Supportive Employment Services (SES)

Customized Community Living Homes

For adults who have more dedicated care needs, we provide 24-hour support services in our customized community living homes. We strive to help our residents maintain their independence in the community while providing:

  • 24-hour staffing
  • Medication management and home health services
  • Housekeeping
  • Socialization and activities
  • Transportation
  • Assistance with shopping, budgeting, cooking, meal planning, making appointments
